While using RMC of grade M-30 or more at site, if at all the time taken to pour the concrete in the forms exceeds 3 hrs, should the concrete in the Transit mixer be rejected or used? Can it be used by adding more cement at site or not?

you should not use that if you respect ACI standard... but in many project tehy use that and usaully it doesn make any problem...
