How to know if your micro SD card is physically damaged?

So I've this micro SD card that refuses to identify itself in variety of operating systems including Windows Vista, Windows 7 and OSX Mountain Lion. When I insert it into my cell phone, Android can't detect it either. So basically, the SD card is almost dead. I assumed that the card needs FAT16 formatting so that Android can recognise it, but that isn't the case. I've a doubt that the card's been physically damaged, but I've no way to find out whether it's actually damaged.

I'd appreciate any inputs in confirming that my micro SD card is physically damaged. There are no apparent scratches or short-connections near the contact plates.
