Advice Needed on implementing a Digital Counter

Hi,
I work in an UK FMCG Company which manufactures Liquid Soaps.I have been particularly assigned a project to improve the efficiency of Liquid Soap Pouching Machine.
The Machine runs on a Chain with Grippers (installed on Sprockets) to clutch the pouches for filling.The problem lies somewhere in the grippers as most of the time they fail to clutch the pouches causing their rejection.
I want to implement a counter which will calculate the number of times a gripper comes without a pouch.
Also, the machine is having 40 sets of Gripper(1set=2 Gripper), so the counter has to mention the rejection happening for each gripper.
