difficulties in harnessing controlled fusion

whats the greatest difficulty we are facing in harnessing controlled fusion.

    The basic problem is how do we confine the plasma which is at the extreme(millions of degrees) hot and we need to confine it to extreme density(nearly to the density of the nucleus(order of 10^18 kg/m).
